Spain's SNS (Sistema Nacional de Salud) is the NHS equivalent — funded by social security contributions, free at point of use for residents. Register at your local centro de salud with your empadronamiento and NIE. Quality is high: Spain ranks 7th globally (WHO). Specialist waits are 2–4 months, often shorter than NHS. The GHIC covers emergency tourist care only — not routine treatment, not prescriptions. The S1 form entitles UK state pensioners to full SNS access — apply via NHS Overseas Healthcare Services before you move. Private insurance (€80–€180/month) is mandatory for non-lucrative and digital nomad visas. Sanitas and Adeslas are the main providers.
UK pension portability
Spain accepts QROPS transfers — several Spanish-compliant schemes exist. Since Spain is in the EU but the UK is no longer EEA-aligned, the 25% overseas transfer charge (OTC) position is complex — transfers to schemes established before Brexit may be grandfathered. Get specialist advice. UK state pension is paid in Spain with full annual uprating (the Withdrawal Agreement protects those registered before 1 Jan 2021; post-2021 movers rely on the UK-Spain bilateral agreement — uprating continues under current UK policy, but the legal protection is less robust). Spain taxes foreign pension income at progressive rates (19–47%) unless the Beckham Law applies (flat 24% for 6 years). UK government pensions (civil service, NHS, armed forces) are taxed only in the UK under the DTA.
Tax residency & double taxation
Spain taxes residents on worldwide income at progressive rates (19–47%). The Beckham Law (Régimen de Impatriados) offers a flat 24% on Spanish-source income up to €600,000 for the first 6 years — available to new tax residents who haven't been Spanish tax residents in the prior 5 years. Digital Nomad Visa holders automatically qualify. The UK-Spain Double Taxation Convention prevents double taxation. UK government pensions: taxed only in the UK. Private pensions: taxable only in Spain once resident. UK rental income: taxed in both countries with credit relief in Spain. Non-resident CGT on your UK property: 18%/24% to HMRC, with credit against Spanish worldwide CGT.
Buying property as a UK national
Non-EU residents can borrow 60–70% LTV from Spanish banks (residents get up to 80%). Post-Brexit, UK buyers are third-country nationals — same category as Americans. No foreign-buyer surtax at national level. Transfer tax (ITP) is 6–10% depending on autonomous community; new-builds attract 10% VAT + 1.5% stamp duty. Non-resident mortgage rates: Euribor + 2–3%. You'll need an NIE (foreigner identification number) before you can do anything — apply at the consulate in the UK or at a police station in Spain. Notary and registry fees add 1–2%. Budget 10–13% on top of the purchase price for total transaction costs.
